Hazel L. Bartlett, born on. August 4, 1963, to the late Birdie Mae Bowers Bartlett and William Henry Bartlett, was a beloved daughter of Louisville, Kentucky, and cherished figure to many, passed away peacefully at her home in Gallatin, Tennessee, on September 16, 2025, at the age of 62. Hazel dedicated her life to caring for others, leaving an indelible mark on the hearts of her family, friends, patients, and colleagues alike.
A proud graduate from the following institutions, she earned her diploma from Sallie Phillips Durrell High School, a Bachelor’s of Science Degree for License Practical Nurse from Lindsey Wilson College of Nursing, and an Associate’s Degree from the College of Education and Human Development at Jefferson Community and Technical College. Hazel blossomed into a vibrant storyteller, writer, play writer, poet, actress, model, musician, artist, songwriter, teacher and mentor. Her many talents resonated deeply with those around her, as she spun enchanting tales and poured her soul into her creative endeavors. Above all, she spend her life nurturing those in her community and family, embodying the essence of compassion and love.
Hazel’s legacy is richly woven into the lives of her living family and friends. She is survived by her loving children: Terriel D. Bartlett, Sr., Stephanie L. Bartlett Lee (Benjamin Sr), Stephen L. Bartlett, Grant E. Morgan II (Jessica), and Matthew B. Morgan (Alexandria Sanders). Her grandchildren - Terriel D. Bartlett, Jr., Terionna Bartlett, Corey Bartlett, Trinity Bartlett, Dashawn Bartlett, Elliott Blunk, Benjamin C. Lee, II, Shania E. Lee, Naomi Morgan, Tiannah Moore, Ryan Moore, and Zechariah Sanders - carry forward her spirit and love. Moreover, Hazel proudly embraced the role of a great-grandmother to two great-grandchildren, enriching her life with even more joy.
She also held a special place in her heart for her siblings, Diane Perry Flora, Vera Perry, Pamela Hammond, Jacqueline M. Bartlett Hollingsworth, and William Keith Bartlett. Alongside them were several nieces and nephews who cherished their time spent with her, learning and growing under her guidance.
Hazel was proud of many accomplishments throughout her life, but one of her greatest was her long-standing friendship with Bill W., a testament to her strength and resilience in the face of life’s challenges.
Hazel is reunited in eternal rest with her parents, William Henry Bartlett and Birdie Mae Bowers Bartlett, her aunt Fannie Geneva, and her brothers, Jerome Perry, Sr., Derwin Sowell (Bartlett), Richard Bartlett, and Steve Harris.
Hazel L. Bartlett’s spirit will forever shine brightly in the hearts of those who had the pleasure of knowing her. Her remarkable journey will be remembered not just for her achievements and creativity, but for the profound love and warmth she radiated to everyone who crossed her path.
A Celebration of Hazel’s life will be held Friday, September 26, 2025 at Sawyer Hayes Community Center, 2201 Lakeland Road, Louisville, Kentucky 40241 from 1:30 - 4:30 P.M.
